REPATRIATION REQUIREMENTS OF HUMAN REMAINS TO ZIMBABWE

1. Death certificate issued by SA home affairs 

2. Copy of deceased’s Zimbabwe passport or birth certificate or identity document (ID) 

3. Copy of deceased relative’s Zimbabwe valid passport or birth certificate or ID. 

4. Contact details of informant and relative in Zimbabwe 

5. Burial order, issued by South African Home Affairs 

6. Embalming certificate, issued by Funeral provider 

7. Non-infectious disease certificate, issued by forensics 

8. Death on arrival certificate for those who died at home issued by paramedics 

9. Health clearance letter from the Ministry of Health and Child Care of Zimbabwe. 

10. Notice of death/still birth – (dha-1663a) BI’s (3) and 1 of 1 

11. Valid certificate of compliance (COC) in the name of the funeral service provider or undertaker.
