REPATRIATION REQUIREMENTS OF UNDOCUMENTED DECEASED ZIMBABWE NATIONALS (R200)

1. Completed Registrar General Department Questionnaire Form/RG 1 form 

2. Zimbabwe national identity card, valid passport and/or Zimbabwe birth certificate of the informant; 

3. Affidavits written by the next of kin and two (2) other relatives in Zimbabwe stating that the person is indeed a Zimbabwean national and should also state the nature of relationship. 

4. For rural burial stamped letters from Chief/Headman/Councillor in Zimbabwe and their contact numbers. 

5. Urban burial (letter from MP or Mayor and Ward Councillor)
