1. Original Birth certificate
2. Original national identity document or valid passport for the father/mother/other legal guardian.
3. Original pay code issued on application
4. Receipts issued on application date
5. Affidavit from the police is required when collecting on behalf of someone.
*VALID PASSPORTS SHALL BE CANCELLED ON THE DAY OF COLLECTION UPON ACKNOWLEDGEMENT OF AVAILABILITY OF THE NEW PASSPORT.
